Permits
Requestor should complete the request form no fewer than 7 days before work is scheduled to start.
This program establishes guidelines to guard against fires from heat-producing operations performed by using portable equipment away from the shop area or any operations that have an open flame (BBQ, cooking, grilling, campfires, bonfires, etc.). Employees and contractors must be aware of procedures to follow when welding, grinding, cutting, or conducting other hot work operations or events.
The instructions for obtaining a Hot Work permit follow.
- Click on the Hot Work Permit Request Form link.
- Complete the request form and click the Update Permit Request button. The form allows document upload if needed. Requestor should complete the request form no fewer than 7 days before work is scheduled to start.
- Shared Services EHS Specialist will receive the request form and contact the requester with any follow-up. Also, EHS will send training modules for hot work participants, including fire watch, to complete before approving the permit.
- The requestor should not begin the hot work until receiving the permit.
- EHS will inspect the hot work operation to assure compliance.
EHS has assessed the risks associated with Departments and Organizations holding potluck meals and has determined published guidelines need to be followed. This permit process assures the organizer is aware of these guidelines.

- Complete the request form and click the Update Permit Request button. The form allows document upload if needed. Requestor should complete the request form no fewer than 7 days before the event is scheduled.
- EHS will receive the request form and contact the requester with any follow-up.
- EHS might decide to inspect the potluck event to assure compliance.
This permit and related procedures set forth the standards and responsibilities for the application and approval of the use of fireworks, pyrotechnics, flame effects, and laser displays at Lamar Institute of Technology / Lamar State College Orange / Lamar State College Port Arthur. These events include, but are not limited to, athletic competitions, ceremonies, conferences, banquet, science demonstrations, parades, or theatrical/concert performances.
- Click on the Fireworks, Pyrotechnics, Flame Effects, and Laser Entertainment Request Form.
- Complete the request form and click the Update Permit Request button. The form allows document upload if needed. Requestor should complete the request form no fewer than 7 days before event is scheduled.
- Risk Management will receive the request form and contact the requester with any follow-up.
- Risk Management will issue the permit before the event begins.
